Active Minds Chapters is a vibrant network of 500+ peer-led, diverse mental health advocacy groups. Our focus is on positive social change, education, advocacy, and creating a brighter, mentally healthier future. Our Chapters offer inclusivity, a sense of belonging, and a supportive community, while intentionally nurturing mental health leaders within their communities.

Your commitment to championing a new era of mental health is inspiring and we’re so thrilled to add your new chapter to our network!

All new and reactivating Active Minds Chapters must complete the Chapter Registration process with the Active Minds National Office. Please review our Chapter FAQs for additional information. 
Note that you must have all information and materials ready to register in order to complete this form.

Once submitted, please allow our team up to two weeks to process your registration form and materials. Your chapter is not officially recognized as an Active Minds Chapter until your registration form has been processed and confirmed by our team.

Please provide a reliable mailing address for your school (not a personal or home address) for our records. All new chapters will receive an Active Minds tablecloth. We will use this address to mail your tablecloth once your registration has been approved.

Chapter Contact Information
Chapters are required to have at least three student leaders, one of whom is not graduating this year, and a faculty or staff advisor. Please provide their contact information.

*Note: By joining Active Minds, you join the Active Minds Chapter Network, through which you will receive emails about important chapter updates and requirements, requests to collaborate, program resources, or funding opportunities. We do not share your contact information with people outside of the Chapter Network without your permission.

Primary Advisor Contact Information
Active Minds Chapters must always have at least 1 faculty or staff advisor, regardless of their school’s rules around advisors for student organizations. Some chapters may choose to have more than 1 advisor. “Primary Advisor” may be your only advisor, or a primary advisor contact if you have more than 1 advisor. You may add another advisor contact here if that is the case for your chapter.

Chapter Expectations
All chapters must read the Expectations of Active Minds Chapters and agree that they understand and will abide by the rules laid out within. You may print and save a copy for your records here

Additionally, Chapter Advisors and Presidents are both required to acknowledge the Expectations on an annual basis moving forward and will be notified via email. 

We hope that by outlining these expectations clearly, your chapter will be better equipped to do the important work of championing a new era of mental health. While this is an important document to establish expectations, it is our intention that it can serve as a helpful resource for your chapter’s operations moving forward to ensure alignment with the National Office.

Documentation of Student Organization Status
We require all chapters to be officially recognized student organizations at their schools.

Please attach documentation of school recognition as an official student group to this form for our team’s review. This documentation can take the form of a letter or email from a Student Activities or administrative staff member with the authority to approve student organizations, minutes from a Student Government meeting where they recognize your chapter, or other official documentation that clearly shows Active Minds has been recognized and approved to operate as a student organization at your school.
